National PACE Association, in partnership with Health Dimensions Group, develops Special Needs Plan (SNP) Resources.

Recognizing the potential impact of Special Needs Plans (SNPs) on PACE programs, the National PACE Association, in partnership with Jade Gong from Health Dimensions Group, has developed Special Needs Plan Resources. The "SNP Resource Kit" is intended to inform and support PACE programs understanding of and efforts to successfully co-exist with SNPs in their marketplace.  The resource kit includes a SNP Primer Presentation, Primer Cover Memo, a Snapshot Comparison of PACE and SNPs and the CMS Final Report, a spreadsheet containing information about SNP sponsors, types of persons served, and the states and counties in which they are to operate in 2006.
