Kathleen Griffin, PhD., Health Dimension Group’s National Director of Post-Acute and Senior Services has been collaborating with, presenting to and meeting with national health care leaders to prepare for the changes expected with health reform. Recently, she was a part of a summit with leaders from health care financing organizations and health care organizations to discuss changes expected in Medicare payment related to avoidable hospital readmissions within 30 days. A significant percentage of these possibly avoidable readmissions are from post-acute care venues.
