Dave Briscoe

Mr. Briscoe is responsible for the coordination of the firm’s business development efforts in the areas of acquisitions and management contracts, developing business strategy, and serving on the board of directors. Mr. Briscoe brings over three decades of health care operations and business development experience to Health Dimensions Group. Mr. Briscoe has particular expertise and skill in managing start-up and turnaround projects.

Erin Shvetzoff Hennessey, MA, NHA, CPG

Ms. Hennessey has expertise in the senior health care industry, advising providers in the areas of post-acute care management, operational and financial performance improvement, and organizational and ownership structure, as well as hands-on experience as an administrator of skilled nursing and senior living facilities in several states. She is active in senior services trade organizations to provide them with education resources, industry information, and support for members, and speaks at state and national meetings on senior health care, health care reform, and planning for aging populations. Ms. Hennessey is a certified gerontologist and served as a guest faculty member on post-acute care for Estes Park Institute, a provider of professional education for hospital executives, board members, and physician leaders. Ms. Hennessey was named a 2019 Senior Living Leader Under 40 by Argentum, a national senior living association.

As president of Health Dimensions Group (HDG), Ms. Rogotzke provides executive leadership of HDG’s operations to direct the organization’s goals, strategy, and business solutions. Previously, Ms. Rogotzke served as HDG’s EVP of Human Resources and is skilled at employee development, human resources best practices, policy and program development, recruitment and retention strategy, benefits and compensation, employee and labor relations, regulatory compliance, and safety, which allow her to develop high-performing leadership teams and engaged workforces.

Sarah Gustafson, CPA, MBA

Sarah Gustafson, EVP, Finance, plans, directs, and controls financial functions for HDG and its managed communities. Ms. Gustafson has a proven track record of improving organizational performance through cost reduction, service line analysis, and performance improvement. Her expertise includes revenue cycle performance, healthcare finance, physician compensation, health plan operations, and healthcare data analysis. Ms. Gustafson is a certified public accountant and holds a bachelor’s degree in accountancy from the University of North Dakota in Grand Forks. She is currently pursuing her master’s degree in business administration from the College of St. Scholastica in Duluth, Minnesota.

Stephanie Signore

Ms. Signore provides expert strategic guidance to the HDG executive leadership team and leads development of marketing, communication, public relations, and professional education strategies and initiatives. With more than 20 years of leadership experience in health care, Ms. Signore has provided direction for strategic marketing efforts at a variety of organizations and systems of up to 35 facilities. She is highly accomplished in the areas of marketing and brand strategy, team development, and provider/partner relationship management.

Sharon Thole
EVP, Operations

Ms. Thole is a seasoned health care operator and business development executive with almost 20 years of experience leading and positioning post-acute and senior living organizations for financial and clinical success and sustainability. As the executive vice president of operations for HDG’s managed communities, Ms. Thole provides guidance to the regional directors and operations teams; interfaces with boards and owners; ensures regulatory compliance; promotes quality care and employee experience; and oversees community financial performance. Her previous experience includes Love Funding’s director of originations for financing of health care facilities and senior housing; COO at a multi-site nonprofit continuing care organization; and operations and business development positions with national skilled nursing and assisted living organizations.

Heather Haberhern

In her role as senior vice president of quality, Heather Haberhern leads the community quality leadership team and provides strategic leadership and oversight for compliance, quality of life, clinical services, clinical reimbursement, plant operations, safety, culinary, and risk management for all managed communities. With in-depth knowledge of the communities’ day to day operations, she implements the Caring Above and Beyond quality and customer experience pillar, oversees Health Dimensions Group’s Enrichment Beyond Expectations quality of life program, and educates staff on the principals of quality improvement. Ms. Haberhern also ensures adherence to state and federal regulations by maintaining current knowledge of regulatory, quality, and clinical trends. She completes reviews and offers recommendations on all incidents of risk and litigation for the managed communities.

Sarah Friede, MBA

Ms. Friede has over 15 years of experience in management and human resources in the senior living setting. She specializes in executing recruitment strategies and leading interim and permanent placements specific to long-term care and senior living communities. Ms. Friede has also provided human resources oversight to Health Dimensions Group’s managed communities and, prior to joining HDG, provided HR support to 13 senior living communities in Minnesota, as well as serving as associate executive director at a 121-bed assisted living and memory care community.

She provides leadership in ongoing development of work processes, systems, and organizational culture, specifically related to recruitment, retention, and development of a superior workforce. Her expertise includes organizational change and development, employee development and training, and employee and labor relations. Ms. Heid’s prior experience includes serving as executive director for a senior living community and senior human resources positions at a variety of organizations. She holds a degree in human resource management and certification as a Senior Professional in Human Resources.

Kim Barnes, RN

Ms. Barnes has almost twelve years of experience in skilled nursing, home health, and behavioral health. She previously served Sentara Life Care as interim VP of clinical operations/nurse executive and has also served as a nursing facility and behavioral health charge nurse and regional manager. Her expertise includes nursing and quality management, and patient advocacy, education, and safety. Ms. Barnes’ prior experience includes regional quality management specialist and risk manager for Adventist Care Centers where she was responsible for all elements of QAPI systems. She also has extensive facility survey experience as surveyor and regional manager with the Kansas Department for Aging and Disability Services.

Ms. Guindon provides regional oversight, leadership, and management of assisted living and independent living communities both owned and managed by Health Dimensions Group. In partnership with HDG’s operational leadership team, Ms. Guindon provides direction in occupancy/revenue management, expense management, safety management, and many other facets of senior living operations. Prior to rejoining HDG, she had operational responsibility for a Minneapolis area health care provider, which included a 320-bed skilled nursing facility, a 194-unit assisted living/independent living community, and an adult day program.

Kathy Karr, RN, NHA

Ms. Karr has nearly four decades of health care experience. Her background includes director of nursing, nursing home administrator, and director of operations, providing leadership for multiple skilled nursing facilities in several states. She provided operations oversight of facility performance and assisted administrators in developing action plans to achieve compliance, as well as business plans and budgets; and served as liaison for investors, boards, and regional support teams.

Tom Stitt

Mr. Stitt has almost 30 years of experience in health care finance and reimbursement, which covers all segments of the health care continuum, including hospitals and health systems, post-acute care, long-term care, home- and community-based services, and PACE. He has become nationally known for his PACE expertise, with over 18 years of experience in the PACE industry as a consultant and a director of finance. In addition to consulting experience, his work experience includes national director of finance, PACE for Ascension Living; cost report auditor at Blue Cross Blue Shield; and director of reimbursement at a large health system.

Colin Higgins
